NOTE REGARDING THE ADA MEDICAL CERTIFICATION FORM: The Employee shall contact the qualified professional  to complete the Employee ADA Medical Certification Form.  Completed forms can be submitted to the ADA/504 Coordinator here. Any Employee ADA Medical Certification Form completed by the Employee, or for which incomplete information is noted, will not be accepted.
