Required Documentation

The UF ADA Office utilizes the Interactive Process to review ADA accommodation requests. Employees seeking an ADA accommodation must obtain medical documentation of the disability and submit the online accommodation request form to initiate the Interactive Process.

Please take note that no accommodation request can be processed without the attachment of a medical provider’s certification of the employee’s disability. The UF ADA Office has provided a Medical Certification Form to support employees in obtaining this information from their licensed medical providers.

Employees must submit both:

  1. Medical Certification Form (PDF) or equivalent letter
  2. Accommodation Request Form

Obtain Medical Certification from Licensed Medical Provider

No accommodation can be processed without medical documentation of the disability.

Physician statement, individual counselor statement, physical therapist statement, and occupational therapist statement are all appropriate and accepted statements.

Exception: For those current employees with visible disabilities (disabilities such as but not limited to, chair user or other devices, visually impaired, hearing impaired/deaf) the accommodation will be provided pending submission of documentation.

The documentation provided by your licensed medical provider must include key information in order to be able to process your request.

  • Verification that the employee has a disability
  • Description of how employee’s limitations impair the ability to perform job duties
  • Indication of whether these limitations are temporary or permanent
  • Recommendation on necessary accommodation(s)

If the letter from your licensed medical provider does not contain these elements you should follow up with them and request a revision.

Submit Accommodation Request to UF ADA

Once you’ve obtained medical certification (completed PDF form or equivalent letter) from your licensed medical provider, attach it to the Accommodation Request form and submit. An ADA Case Manager will receive and review the request.
